Project H.O.P.E. offers free health screenings, health education, wellness services, preventative care and referrals to residents living in the surrounding community to help them identify and manage chronic conditions before they significantly impact their quality of life.

With the help of our partner organizations, we provide services such as:

Every time we have the opportunity to connect in person with individuals, whether it be at one of our events or at a scheduled appointment at our office, we make sure to take the time to perform a general screening to assess their physical and social needs. This includes taking their blood pressure, reviewing medications, assessing health maintenance routines, and providing education as the need arises.  At events, these services are provided alongside or in conjunction with the planned care for the day.

When we identify individuals with health problems at our events, we assist them in overcoming whatever obstacles they may be facing to take control of their health and ensure their continuity of care.  Some common barriers we encounter include the lack of financial support to seek additional care, the inability to travel to appointments, and a lack of knowledge regarding the health care resources available in the community.

To further assist in addressing health care service gaps, Project H.O.P.E. is able to provide low-income, uninsured or underinsured adults with financial assistance to obtain prescription medications, vision care (eye exams/glasses), dental care, appointments at neighborhood clinics, and bus transportation to medical appointments.

In addition to caring for the physical needs of each individual, Project H.O.P.E. offers spiritual support, prayer, and a comforting presence rooted in the Catholic tradition.

During the covid-19 pandemic, we are continuing to provide care for the community. We are reaching out to individuals over the phone to check in on their mental health, review their needs, and review health data such as blood pressures taken on personal machines, and blood glucose levels. Deliveries of food and supplies are being organized when possible to help ensure that everyone stays healthy and safe.
