We address the obstacles neighbors in the Cleveland community are facing when trying to access health care and maintain their personal well-being and a sense of dignity. In the long-term, Project H.O.P.E.'s goal is to prevent catastrophic outcomes from untreated chronic illness and to treat acute illnesses promptly before the condition severely limits the individual's quality of life or becomes life threatening.
